Nice to be appreciated

Of course I passed on thanks to everyone involved . I fully aware we could not have done this without every member of the Multi Disciplinary Team . Other members got cards too . I just thought it was nice of the relative . I think we bonded more closely because of my Dad & his fight with dementia . But , we have a fantastic team & are very close knit

I hope you passed on the thanks to all the other tens of people who worked so hard and made this patients discharge possible and safe -other nursing staff, the doctors, support staff, physios, occupational therapists, mental health team, pharmacy staff, specialists nurses and DRT staff who are often 'behind the scenes' staff but are just as, if not more, vital to the process but often forgot by relatives.

A successful discharge is never a one person achievement.

Agree with your comments, when a patient is ready for discharge, they are not allowed home without a full assesment which isn't done by the nurse but by the Multi Dis. Team, perhaps what the relative was thanking the nurse for was the care given when she was on duty (along with rest of the staff on the ward)

A discharge plan is done by the Nurses along with the rest of the team , it is of course up to the Consultant caring for the patient to deem them fit for discharge then the rest of the team start the discharge process. Safety is paramount or we will have all failed in our Duty of Care.

This mans daughter , as I have said before wanted to thank me because I understood what was happening to her Dad with regards to his dementia .
