Guest Posted March 7, 2008 Share Posted March 7, 2008 Completed. Couple of issues though: You should have a look at your questions. You've made answering all of them mandatory, even when a response isn't applicable. Not a problem for the questions where you have to type something - you can enter 'N/A' in the text box - but question 10 doesn't have a tick box for 'don't want to lose weight'. You also have a couple of spelling/punctuation errors: Question 3: 'your' not 'you'. Question 8: 'you're' not 'your'.
