Stray cat frequenting my garden

I've seen a white longhaired cat on and off for a couple of months in my back garden it never really caught my eye fully before, in the past couple of weeks it has been coming around the house more and more and is looking unkempt and pretty skinny. It doesn't have a collar and is nervous if you approach it, I presume it's a stray but not sure how to catch it to take it to the vets as it really doesn't look healthy. Any advice or ideas would be helpful... I have two cats my self and there are many cats that live around me also.
