Follow the instructions here to the letter (Automated Removal Instructions for Antimalware Doctor, steps 1 to 16), using Rkill and Malwarebytes.

If that doesn't work, post back and say at which step you have problems with.

No, don't use Spyware Doctor.

Scroll down about half way down the page to where it says:

Automated Removal Instructions for Antimalware Doctor using Malwarebytes' Anti-Malware:

  1. Print out these instructions as we may need to close every window that is open later in the fix.

  2. It is possible that the infection you are trying to remove will not allow you to download files on the infected computer. If this is the case, then you will need to download the files requested in this guide on another computer and then transfer them to the infected computer. You can transfer the files via a CD/DVD, external drive, or USB flash drive.

  3. .....

....and follow steps 1 - 16

The program Rkill will stop the antimalware doctor processes running which should in turn allow you to run Malwarebytes because otherwise antimalware doctor will block it and stop it running.

Malwarebytes' Anti-Malware to give it it's proper name. It's important to get it right as there are so many rogue programs that have similar names that try to trick people into downloading them. If people search the wrong thing they could end up with something they really didn't want. ;)
