They come from separate places.

Your application for a blue badge goes through the council, somewhere in Howden House (they're in the phone book or online Council directories).

Your application for free road tax you first need to ring the DLA people in Blackpool and ask for a certificate stating that you qualify for free road tax. You qualify if you get high rate mobility portion of DLA.

When that arrives you then need to change your vehicle over to 'Disabled' taxation class in order to show a 'disabled' tax disc, so you need to take your v5 for your car, your certificate of eligibility, MOT certificate, proof of insurance and your current tax disc to the local DVLA office which is just off the town end of Ecclesall Road (again, in the phone book etc).

When you're there you'll need to fill in a form to change the excise class of your vehicle (there are piles of all of the regular forms at the back of the room there), take a ticket and wait for however long it takes to be seen and they will surrender your current tax disc (you'll get a cheque through the post, not refunded on the day), issue you with a new tax disc and send off your v5 to change the taxation class on your v5 for future use.

Once you've gone through that process with that vehicle you can then apply for tax discs online from then on, as long as your taxation class doesn't need to change back again (which takes the same process as above at the DVLA office).

You do need to be aware that you have to do the process in reverse whenever you sell the vehicle if the person you're selling it to doesn't get free road tax.
