Go to this page. (http://www.inlandrevenue.gov.uk/nic/ynino.htm#9)
Scroll down to the bottom and you will see the answer to your question.
Everybody tends to have a different tax office depending upon where they first registered. The odds are in favour of yours being the South Yorkshire office in Sheffield.

Follow the links on that page to find your tax office to contact.

The phone number for Sheffield is 0845 366 7802

I changed my name by deed poll and you have to send a copy of the poll to the main NI office - I'll see if I still have the address somewhere :)

Do You Need To Know If I Move Or Change My Name?

Yes we do. Please write to your nearest Social Security office (This is a link to the DWP (formerly DSS) website) or Inland Revenue (NI Contributions) office quoting your NI number. Tell us what change has occurred e.g., new address or change of name and when it happened.

If you do not tell us when you move, marry or otherwise change your name we will not be able to keep your records up to date. This means we will be unable to contact you if we need to; for example, if the contributions you paid in a tax year are not enough for that year to count for benefit purposes. We would normally write to let you know and to tell you how much you could pay in voluntary contributions to make that year count.

If you change your name, please return your old NINO card to your nearest Social Security Office when applying for a card in your new name.

See the question on this page (http://www.inlandrevenue.gov.uk/nic/ynino.htm#9) about changes to your name - you can then enter your area to get the right address to send the deed poll to. For Sheffield :

Concept House 5 Young Street SHEFFIELD S1 4LA

EDIT : Cgksheff beat me to it :P
