Ever read a story by Steven King called 'Survivor Type'? It adresses this very subject - a junior surgeon smuggling a load of heroin is shipwrecked on an island, there's nothing to eat, and whilst trying to hunt a seagull he breaks his ankle.
Using the smack as anasthetic, he performs surgery on himself, and ends up chopping bits off to eat, the story is written in the form of his diary, and ends when he gets round to his fingers.
