Hi.

I suffer from hypermobility syndrome which means I have chronic pain, and I have been to talk to my GP today about some more physiotherapy as my condition has deteriorated since having my daughter 2 years ago. I asked her about what sort of support I might be able to get, and she seemed quite positive about the possibility of me getting a mobility pass for trams/buses. She said she'd write me a supporting letter to help, but didn't tell me who I have to ask for a pass! What is the process?

I also asked about DLA and while she stressed that they've made it much more difficult to get, she said it was worth asking and that I could probably use the same letter. However, she didn't tell me who I need to speak to about that either...

So, who is in charge of these things and what would I need to show/prove to get them? :help:

You have to go to the COuncil, get a form, take it back to your GP, get your GP to sign it and provide evidence of how your disability affects you. The form provides all the information you need and it's available at Howden House reception

Krissy,

You can get DLA forms from the DWP, phone them up, and ask for them to be sent to you. They'll send them out to you, giving a date when they need to be sent back for. (they give you something like six weeks to return them)

My advice to you is not to try and fill them in yourself, but ask your local CAB for assistance. You have a facility where you can book a longer appointment at my local CAB if it's a DLA form. It helps, to have someone in the know assisting you to fill in the form, becasue they can be more specific, rather than vague, which could mean the difference between a successful claim, and a failed claim

Be warned, they are a nightmare to plough through; the forms are so long, especially if you suffer fatigue.

Best of luck in your claim, anyway.

number to ring for a dla form is 0845 123456. You get six weeks to send it in from the date stamped on the front page and then if yolu are successful in getting it you get it backdated to the same date. Good luck
